§ 4.97 Schedule of ratings—respiratory system.

Blastomycosis

A fungal infection that may affect the lungs and cause cough, fever, and breathing difficulty.

What the rating means

Blastomycosis is rated under the General Rating Formula for Mycotic Lung Disease, which covers diagnostic codes 6834 through 6839. The formula has four possible ratings based on how active the disease is and what symptoms you have.

A 100% rating applies when you have chronic pulmonary blastomycosis with persistent fever, weight loss, night sweats, or massive hemoptysis. A 50% rating applies when the disease is chronic and requires suppressive (ongoing, long-term) therapy, with no more than minimal symptoms such as occasional minor hemoptysis or productive cough. A 30% rating applies when you have chronic pulmonary blastomycosis with minimal symptoms, such as occasional minor hemoptysis or productive cough, but suppressive therapy is not required. A 0% rating applies when the mycotic lesions have healed and are inactive and you have no symptoms.

The key distinctions are how active the disease remains, whether serious constitutional symptoms are present, and whether ongoing suppressive therapy is needed to control the infection.

Evidence that helps

Medical records documenting the course of your blastomycosis are central to showing where your condition falls in the rating formula. For the 100% level, records showing persistent fever, documented weight loss, night sweats, or massive hemoptysis help establish that rating. Temperature logs, laboratory results, imaging, and pulmonologist or infectious disease notes that describe the severity and duration of these symptoms are all relevant.

For the 50% level, records showing that a provider has prescribed suppressive antifungal therapy on a long-term basis, along with notes describing minimal symptoms such as occasional minor hemoptysis or productive cough, support that rating. Pharmacy records or treatment plans confirming ongoing medication use can help here.

For the 30% level, records showing minimal symptoms without a requirement for suppressive therapy are relevant. For any active rating, your own statements describing how symptoms such as coughing, bleeding, fever, or fatigue affect your daily activities and ability to work can help paint a full picture of your condition alongside the clinical findings.

Secondary conditions

Conditions that are often caused or made worse by this one. If you have any of these, they may be claimable as secondary service connection.
