6519
§ 4.97 Schedule of ratings—respiratory system.

Complete loss of voice (organic aphonia)

Complete loss of voice due to physical damage or disease of the vocal structures.

What the rating means

VA rates complete organic aphonia under Diagnostic Code 6519 in the respiratory system schedule. The rating reflects how severely your ability to communicate by voice is affected.

A 100% rating applies when you have a constant inability to communicate by speech. A 60% rating applies when you have a constant inability to speak above a whisper. The word "constant" matters at both levels — the loss of voice must be persistent, not occasional.

If your aphonia is incomplete rather than complete, VA does not rate it under this code. Instead, VA evaluates incomplete aphonia as chronic laryngitis under Diagnostic Code 6516.

Evidence that helps

To support a 100% rating, evidence would need to show that you are constantly unable to communicate by speech at all. Relevant records include laryngoscopy or other diagnostic findings confirming a complete organic cause, physician notes documenting total loss of voice across multiple visits, and statements from you or people who know you describing that you cannot speak under any circumstances.

To support a 60% rating, evidence would need to show that you are constantly unable to speak above a whisper. Medical records noting persistent severe voice limitation, exam findings showing significantly reduced or absent vocal cord function, and statements describing your day-to-day inability to raise your voice would all bear on this level. Records showing how the whispering-only communication affects your work, relationships, or daily activities can help illustrate the constant nature of the limitation.
