§ 4.71a Schedule of ratings—musculoskeletal system.

Forearm amputation below mid-forearm

Amputation of the forearm below the mid-forearm level.

What the rating means

Diagnostic code 5124 covers amputation of the forearm below the insertion of the pronator teres muscle, which places the amputation in the lower portion of the forearm. VA rates this condition at 70% if the amputation is on your major arm — meaning your dominant side — and 60% if it is on your minor arm, meaning your nondominant side. There is only one rating level for each side; the percentage does not increase or decrease based on how far below the pronator teres the amputation occurs. If you have lost parts of both arms, VA may also evaluate your combined losses under a separate table for multiple extremity losses set out in 38 CFR 3.350, which can affect the overall rating assigned.

Evidence that helps

To support a rating under this code, medical records need to document the exact level of the amputation and confirm it falls below the insertion of the pronator teres. Surgical records, operative reports, or pathology notes from the time of amputation are the clearest way to establish that. A VA or private examination that identifies which arm was amputated — and confirms whether that arm was your dominant or nondominant side — is central to determining whether the 70% or 60% rate applies. Statements from you or people who know you describing how you used each hand before the amputation can help establish dominance. Records showing residual limb condition, prosthetic use, and how the loss affects work tasks and daily activities may also be relevant to any related evaluations.

Secondary conditions

Conditions that are often caused or made worse by this one. If you have any of these, they may be claimable as secondary service connection.
