§ 4.71a Schedule of ratings—musculoskeletal system.

Forefoot amputation

Amputation of the forefoot with loss of more than half of the metatarsal bones.

What the rating means

Diagnostic code 5166 covers amputation of the forefoot at a point proximal to the metatarsal bones, meaning more than one-half of the metatarsal bones have been lost. There is one rating for this condition: 40%. Because there is only one rating level, VA is not distinguishing between varying degrees of severity within this code. The 40% rating applies regardless of which foot is affected. If you have lost portions of both feet at this level, VA may also consider special monthly compensation provisions under separate rules governing multiple extremity losses, but the single-foot rating under this code remains 40%.

Evidence that helps

To support a 40% rating under this code, the key evidence is documentation confirming that an amputation occurred proximal to the metatarsal bones with loss of more than one-half of the metatarsals. Operative reports or surgical records describing the level of amputation are central. Imaging such as X-rays can confirm the extent of bone loss. A VA or private medical examination that measures and describes the residual limb, including exactly where the amputation was performed relative to the metatarsal bones, directly addresses the requirement. Records showing the use of a prosthesis or assistive device, along with statements from you or people who know you about how the amputation affects walking, standing, and daily activities, can also support the overall picture of the disability.
