§ 4.124a Schedule of ratings—neurological conditions and convulsive disorders.

Hypoglossal nerve neuritis

Inflammation of the hypoglossal nerve that may cause tongue pain or difficulty with speech and swallowing.

What the rating means

Hypoglossal nerve neuritis is rated under diagnostic code 8312, which applies the same rating criteria used for paralysis of the hypoglossal nerve (code 8212). VA rates this condition based on the degree of lost or impaired motor function of the tongue. Three ratings are possible: 50% for complete paralysis, 30% for incomplete paralysis that is severe, and 10% for incomplete paralysis that is moderate. Because this condition is neuritis rather than paralysis, the regulation defines "incomplete paralysis" as a degree of lost or impaired function substantially less than complete paralysis. When the involvement is wholly sensory, the rating should be for the mild or, at most, the moderate degree. The ratings apply to one side; if both sides are affected, VA combines the separate ratings using the bilateral factor.

Evidence that helps

Medical records that document the extent of tongue motor dysfunction are central to showing where your condition falls on the rating scale. A VA or private examination that directly tests tongue movement, strength, and coordination can show whether loss of function is complete or incomplete, and whether incomplete loss is severe or moderate. Imaging, nerve conduction studies, or clinical notes describing the progression of your symptoms over time can also help establish severity. If your involvement is purely sensory with no motor loss, that points toward the lower end of the scale. Statements from you or people who observe you daily — describing how tongue weakness affects speaking, chewing, or swallowing — can help show functional impact. Records from speech-language pathologists or other treating providers who have assessed tongue function add objective detail that ties directly to the rating criteria.

Secondary conditions

Conditions that are often caused or made worse by this one. If you have any of these, they may be claimable as secondary service connection.
