5109
§ 4.71a Schedule of ratings—musculoskeletal system.

Loss of use of both hands

Complete loss of function in both hands without anatomical loss.

What the rating means

VA rates loss of use of both hands under diagnostic code 5109 at a single rating level. If you have loss of use of both hands, VA assigns a 100% rating. There are no lower rating levels under this code — the condition either meets the standard for loss of use or it does not. Loss of use does not require that both hands be physically absent; it means the hands no longer function in a way that allows any practical use. Because this condition involves both hands together, VA evaluates it as a single combined disability rather than rating each hand separately under this code. You may also be eligible for special monthly compensation under 38 CFR 3.350, which addresses certain combinations of extremity losses, but that determination follows a separate process outside the rating schedule itself.

Evidence that helps

To support a 100% rating under diagnostic code 5109, evidence needs to show that you have lost effective use of both hands. Medical records documenting the cause of the loss — such as traumatic injury, nerve damage, amputation, or severe disease affecting both hands — are central. VA examination findings that describe the actual function remaining in each hand carry significant weight. Physicians' notes recording that you cannot perform grasping, pinching, or other purposeful hand movements help establish the threshold. Imaging, nerve conduction studies, or surgical reports can corroborate the severity. Statements from you or people who know you that describe how the condition affects daily tasks — such as dressing, preparing food, or holding objects — show the practical impact on function. Records from occupational therapists or rehabilitation specialists that document functional limitations can also reflect the degree of loss that VA's standard requires.

Secondary conditions

Conditions that are often caused or made worse by this one. If you have any of these, they may be claimable as secondary service connection.
