§ 4.88b Schedule of ratings—infectious diseases, immune disorders and nutritional deficiencies.

Q fever

A bacterial infection transmitted from animals, causing fever, fatigue, and flu-like symptoms.

What the rating means

Q fever (Coxiella burnetii infection) is rated under the General Rating Formula for Infectious Diseases. While your disease is active, it is rated at 100 percent. Once the active disease has resolved, the infection itself is rated at 0 percent. However, Q fever can leave lasting health problems, and VA rates those residual conditions separately under whichever body system they affect. The regulation lists specific examples of residuals that may apply: chronic hepatitis, endocarditis, osteomyelitis, post Q-fever chronic fatigue syndrome, and vascular infections. Each of those conditions would be evaluated under its own diagnostic code and rating criteria within the appropriate body system.

Evidence that helps

During the active phase, medical records showing a confirmed diagnosis of Coxiella burnetii infection, positive laboratory tests, and clinical findings of ongoing illness support a 100 percent rating. Once active disease resolves, the evidence that matters most is documentation of specific residual conditions. For example, liver function tests and biopsy results support chronic hepatitis; echocardiograms and cardiology records support endocarditis; imaging and bone culture results support osteomyelitis; and records documenting persistent, debilitating fatigue support post Q-fever chronic fatigue syndrome. Statements from you or those who know you describing how residual symptoms affect your work and daily activities can also help document the severity of each residual condition under the body system where it is rated.

Secondary conditions

Conditions that are often caused or made worse by this one. If you have any of these, they may be claimable as secondary service connection.
