7202
§ 4.114 Schedule of ratings—digestive system.

Tongue loss

Partial or complete absence of the tongue due to surgical removal, injury, or congenital condition.

What the rating means

VA rates loss of the whole or part of the tongue under diagnostic code 7202. Three ratings are possible: 100%, 60%, and 30%.

The 100% rating applies when you cannot take in any nutrition by mouth at all. The 60% rating applies when you can still eat and drink by mouth but have a permanently impaired ability to swallow that requires a dietary modification prescribed by a medical provider. The 30% rating applies when you can still eat and drink by mouth and have a permanently impaired ability to swallow, but no prescribed dietary modification is required.

If tongue loss also affects your speech, VA rates that speech impairment separately — either as complete organic aphonia under diagnostic code 6519, or as chronic laryngitis under diagnostic code 6516 if the loss is incomplete. Note that diagnostic code 7202 is not among the digestive system codes that must be combined into a single rating; those combination rules apply to diagnostic codes 7301–7329, 7331, 7342, 7345–7350, 7352, and 7355–7357.

Evidence that helps

For the 100% rating, medical records documenting that you cannot take any nutrition by mouth — such as records showing dependence on a feeding tube or other non-oral nutrition — directly support that level.

For the 60% rating, records showing a permanent swallowing impairment are key, along with a written prescription or order from a medical provider specifying a dietary modification required because of your tongue condition. This could include a swallowing study, imaging, or notes from a speech-language pathologist, paired with the provider's dietary prescription.

For the 30% rating, similar records showing permanent swallowing impairment apply, but here the evidence would reflect that no prescribed dietary modification has been required.

Across all levels, statements from you or people who know you describing how your swallowing difficulties affect eating, daily activities, and work can help show the severity of the functional impact. Because dietary modifications must be prescribed by a medical provider to count toward the 60% level, documentation of that prescription is essential at that rating tier.

Secondary conditions

Conditions that are often caused or made worse by this one. If you have any of these, they may be claimable as secondary service connection.
