§ 4.97 Schedule of ratings—respiratory system.

Total laryngectomy (voice box removal)

Complete surgical removal of the larynx (voice box).

What the rating means

Total laryngectomy — the surgical removal of the entire voice box — is rated at 100% under diagnostic code 6518. There is only one rating level for this condition; if you have had a total laryngectomy, the rating is 100%. No lower ratings exist under this code.

If your surgery was a partial laryngectomy rather than a total one, VA does not rate it under this code. Instead, VA rates the residual effects of a partial laryngectomy under separate diagnostic codes: laryngitis (DC 6516), aphonia (DC 6519), or stenosis of the larynx (DC 6520), depending on which lasting symptoms you have. The rating your partial laryngectomy receives depends on which of those conditions best captures your remaining impairment and how severe it is.

Evidence that helps

Because the sole rating under this code is 100%, the key evidence is documentation that a total laryngectomy was performed. Operative reports, surgical pathology records, and hospital discharge summaries that confirm complete removal of the larynx are the most direct forms of evidence. A VA or private physician's statement identifying the procedure as a total laryngectomy — rather than a partial one — can also help establish which diagnostic code applies.

If your procedure was a partial laryngectomy, the evidence needed depends on which residual condition VA uses to rate you. For laryngitis or aphonia, records showing ongoing voice impairment or voice loss matter. For laryngeal stenosis, imaging or scope findings that document narrowing of the airway are relevant. Statements from you or people who know you that describe how your voice or breathing difficulties affect your work and daily activities can support the picture drawn by clinical records.

Secondary conditions

Conditions that are often caused or made worse by this one. If you have any of these, they may be claimable as secondary service connection.
