§ 4.79 Schedule of ratings—eye.

Visual pathway disorders behind optic chiasm

Damage to the visual pathways in the brain behind the optic chiasm, affecting vision processing.

What the rating means

Post-chiasmal disorders (diagnostic code 6046) are rated under the General Rating Formula for Diseases of the Eye. VA evaluates this condition in two ways and assigns whichever produces the higher rating: by the visual impairment the condition causes, or by the number of incapacitating episodes you had in the past 12 months.

For the incapacitating-episode path, an incapacitating episode means your eye condition was severe enough that you needed a clinic visit specifically for treatment. The ratings based on treatment visits are 60% for 7 or more visits, 40% for at least 5 but fewer than 7 visits, 20% for at least 3 but fewer than 5 visits, and 10% for at least 1 but fewer than 3 visits.

For the visual-impairment path, VA applies the visual impairment rules found in 38 CFR 4.75 through 4.78 and in diagnostic codes 6061 through 6091 within § 4.79. The rating VA assigns is whichever of the two paths yields the higher percentage.

Evidence that helps

If your rating is based on incapacitating episodes, the most important records are those that document each clinic visit made specifically to treat your eye condition within the past 12 months. Treatment examples noted in the regulation include systemic immunosuppressants or biologic agents, intravitreal or periocular injections, laser treatments, and other surgical interventions. Visit logs, treatment notes, and appointment summaries that show the date, provider, and purpose of each visit directly support the visit count at each rating level.

If your rating is based on visual impairment, the relevant evidence is whatever is required under 38 CFR 4.75 through 4.78 and diagnostic codes 6061 through 6091, such as measured visual acuity and visual field test results from an eye examination.

Your own statements and those of people who know you can also describe how episodes affect your work and daily activities, which helps establish the severity and frequency of the condition. Records from every treating provider, including specialists, help build a complete picture of your treatment history.
