Your Social Security number; if you are not a U.S. citizen, your alien registration number, work permit type, or passport number and expiry date; the name, mailing address and phone number of your last employer and your dates of employment; the hours and gross wages you earned in your last week worked; the gross holiday, vacation, severance or bonus pay you received that week; your SF8 and SF50 if you were a federal employee in the last 18 months; your DD214 member copy 4 if you were a service member in the last 18 months; and your retirement pension information if you have one.
