Submit the completed Application for Military Skills Test Waiver, including certification by your commanding officer, to any Arizona Department of Transportation CDL office.

• You are serving on active duty in the U.S. Armed Forces or separated within the last year under honorable conditions
• You operated a commercial motor vehicle similar to the class you are applying for during the two years immediately before discharge or before applying
• You have not held more than one license, other than a military license, in the past two years and have had no license suspended, revoked or cancelled in that time
• You have no convictions for the disqualifying offenses listed on the application, no more than one conviction for the listed serious traffic violations, and no at-fault accident or traffic-control conviction arising from a crash
