Arizona Gold Star Military Medal

One commemorative state medal issued to the next of kin

Arizona awards the Gold Star Military Medal to the family of an Arizona resident service member killed in action on or after February 13, 1912. Qualifying deaths are those in action against an enemy of the United States, in military operations involving conflict with an opposing foreign force, while serving with friendly foreign forces in an armed conflict the United States is not a party to, or while serving in a combat zone designated by presidential order. Only one medal is issued to the next of kin for each qualifying service member.

How to apply

Send the completed application with your documentation to Arizona Gold Star Military Medal, c/o Arizona Department of Veterans’ Services, 3839 N 3rd Street, Phoenix, AZ 85012.

What you'll need

The completed application, a DD1300 Report of Casualty, and proof of your family relationship to the service member.

Eligibility

  • You are a family member of an Arizona resident service member who was killed in action on or after February 13, 1912
  • The death occurred in action against an enemy of the United States, in military operations involving conflict with an opposing foreign force, while serving with friendly foreign forces in an armed conflict the United States was not a belligerent party to, or while serving in a combat zone designated by presidential order
