Arizona MVD offers 26 distinctive license plates honoring service members, veterans, retired service members and eligible family members. Part of the money from each plate goes to a fund supporting Arizona veterans and their families.

How to apply

Apply online by clicking the plate image in the MVD Specialty and Personalized Plates Gallery, in person at an MVD field office or third-party vendor that processes plate transactions, or by mailing the special plate application to Mail Drop 801Z, Special Plates Unit, Motor Vehicle Division, PO Box 2100, Phoenix AZ 85001-2100.

Proof of eligibility for the specific plate you want, which is usually a DD214, military ID card or veteran service organization ID card.
