Mail the completed and notarized MVD Travel Licenses/Identification Application with the required copies and a $7.00 check or money order to Arizona Motor Vehicle Division, MD510M, PO Box 2100, Phoenix, AZ 85001.
The notarized application, copies of the front and back of your current driver license and of your military ID card, a copy of your Motorcycle Safety Course completion card, and a $7.00 check or money order.
