If you are an Arizona National Guard member and you are injured, become ill, become disabled or die in the line of duty, you and your family are covered for medical care and compensation under Arizona workers’ compensation law. Workers’ compensation will not pay when similar benefits are already provided under federal law or regulation.

Report the injury or illness through your chain of command and file a workers’ compensation claim through the Arizona Department of Emergency and Military Affairs.

What you'll need

Your duty orders, a line of duty determination, and your medical records for the injury or illness. For a death claim, the death certificate and proof of your relationship to the service member.

Eligibility

  • You are an Arizona National Guard service member, or the survivor of one
  • The injury, illness, disability or death happened in the line of duty
  • You are not already receiving similar benefits under federal law or regulation
