Arizona State Veteran Homes

Arizona Department of Veterans’ Services (ADVS)
Medicare-certified long-term and rehabilitative care at four state homes

Arizona runs four Medicare-certified state veteran homes providing long-term and rehabilitative care: ASVH Phoenix, a 104-bed skilled nursing facility at 4141 North Silvestre Herrera Way; ASVH Tucson, 120 beds at 555 East Ajo Way; ASVH Yuma, 80 beds at 6051 E 34th Street; and ASVH Flagstaff, 80 beds at 2100 N Gemini Drive. They offer skilled nursing, professional therapies, recreational programs and social support. Spouses and Gold Star Parents can be admitted too.

How to apply

Apply online, print a hard copy application, or ask for an application packet to be mailed to you from the ASVH admissions page. For help, call the admissions hotline at 602-234-5678.

What you'll need

Your DD214 or other discharge paperwork, a physician’s determination that you require skilled nursing care, and evidence you do not have tuberculosis.

Eligibility

  • You are a veteran discharged from the U.S. Armed Forces under conditions other than dishonorable, or the spouse of one, or a Gold Star Parent
  • A physician has determined that you require skilled nursing care
  • You can provide evidence that you do not have tuberculosis
