Long-term, rehabilitation, memory, respite, assisted living and hospice care at five state homes

Colorado runs five Veterans Community Living Centers offering long-term skilled nursing care, short-term Medicare-certified rehabilitation after a hospital stay, memory care in secure units at Fitzsimons, Florence, Rifle and Walsenburg, short-term respite care at Florence, Homelake, Rifle and Walsenburg, assisted living in 48 renovated cottages at Homelake, and end-of-life and hospice care at all five. The homes are at Fitzsimons in Aurora, Bruce McCandless in Florence, Homelake in Monte Vista, Rifle, and Spanish Peaks in Walsenburg.

Download the admission forms for the home you want from the Admission Forms for Individual Facilities tab on the Veterans Community Living Centers website, then contact that home directly. What you'll need

The veteran’s DD214 showing an honorable discharge, medical documentation of the care you need, and for a family member, proof of your relationship to the veteran.

Eligibility

  • You are an honorably discharged veteran, the spouse or surviving spouse of one, or the parent of a service member who died while serving in the U.S. Armed Forces
  • You need the level of care the centre provides
