Apply through the Connecticut Department of Motor Vehicles Special Order Plate Unit, 60 State Street, Wethersfield, CT 06161. The plate you want determines which application and proof of eligibility you send.
Proof of eligibility for the specific plate, usually your DD214, military ID, organization membership or family documentation, plus the fee listed on the application.
