Death Benefits for Pennsylvania National Guard Service Members Killed in the Line of Duty

Pennsylvania Department of General Services, Bureau of Finance and Risk Management
Monthly income benefit equal to the deceased member’s monthly salary, adjusted annually by the Consumer Price Index

If a Pennsylvania National Guard member dies from injuries received in the line of duty, their beneficiaries receive a monthly income benefit equal to the member’s monthly salary, adjusted each year by the Consumer Price Index. All claims have to be submitted within three years of the date of death.

How to apply

Mail the claim for death benefits of a public safety worker, the report of death, and everything on the Acts 101 and 51 claim processing checklist to the Department of General Services, Bureau of Finance and Risk Management, Room 406 North Office Building, Harrisburg PA 17125. What you'll need

The completed claim and report of death forms, the death certificate, a line of duty determination, proof of the member’s monthly salary, and proof of your relationship to them.

Eligibility

  • You are a beneficiary of a Pennsylvania National Guard service member
  • The service member died from injuries received in the line of duty
  • You submit the claim within three years of the date of death
