You can apply for skilled nursing care at a 144-bed state long-term care facility in Milford, Delaware. It provides 24-hour professional nursing care, medical services, physical, occupational and speech therapies, social services, a library and computer with internet access, a gift shop, a barbershop, and multi-denominational chapel services. Short-term and respite care are also available.

How to apply

Contact the admissions office at 302-424-8572 and email your completed admissions application to DOSDVH_admissions@delaware.gov. There are separate applications for long-term care and for respite or short-term care.

What you'll need

Your DD214 or other discharge paperwork, proof you have lived in Delaware for at least a year, medical documentation that you require skilled nursing care, and your relationship documentation if you are applying as a surviving spouse or parent.

Eligibility

  • You have been a Delaware resident for at least one year before you apply
  • You require skilled nursing care
  • You served at least 180 days on active duty and received an honorable discharge, or you are a National Guard or Reserve member eligible for retirement pay at age 60, or you are a National Guard member with at least 180 days of active duty and an honorable discharge, or you are a Reserve member with at least 181 days of active duty and an honorable discharge, or you are the surviving spouse or parent of a service member or veteran who died from a service-connected cause
