What you'll need

The service member’s state active duty orders, the death certificate, the line of duty determination, and proof of your relationship to the service member.

Eligibility

  • You are a beneficiary of a Florida National Guard service member
  • The service member was killed, or died from injuries received, while serving on state active duty
