Florida State Veterans’ Nursing Homes and Assisted Living Facility

Nine state-run long term care facilities, eight skilled nursing and one assisted living

FDVA runs eight skilled nursing homes and one assisted living facility, all licensed and inspected by the VA and the Agency for Health Care Administration, and supervised around the clock by registered and licensed nurses. The assisted living facility is the 150-bed Robert H. Jenkins Jr. Veterans’ Domiciliary Home in Lake City. The skilled nursing homes are Emory L. Bennett in Daytona Beach (120 beds), Baldomero Lopez in Land O’ Lakes, Alexander "Sandy" Nininger in Pembroke Pines, Clifford C. Sims in Panama City, Douglas T. Jacobson in Port Charlotte, Clyde E. Lassen in St. Augustine and Ardie R. Copas in Port St. Lucie (120 beds each with a 60-bed dementia and Alzheimer’s unit), and Alwyn C. Cashe in Orlando (112 beds with a 32-bed dementia and Alzheimer’s unit). Spouses may be admitted if space allows.

How to apply

Complete the FDVA State Veteran Nursing Home Application Packet and send it to the home you want. The Robert H. Jenkins Jr. Domiciliary Home uses its own separate application packet.

What you'll need

Your DD214 showing an honorable discharge, proof of Florida residency before admission, and certification from a VA physician that you need skilled nursing or assisted living care.

Eligibility

  • You received an honorable discharge
  • You are a legal Florida resident before admission
  • A VA physician certifies that you require skilled nursing care or assisted living care
  • If you are applying to the Robert H. Jenkins Jr. Domiciliary Home, you can feed and dress yourself and need assisted living rather than skilled nursing care
