How to apply

Submit the completed application and your documentation in person at an Ohio Bureau of Motor Vehicles Deputy Registrar License Agency, or by mail to the address on the application.

What you'll need

If you are a veteran, a letter from the VA dated within the past year confirming your disability is service-connected. If you are on active duty, a convalescent leave form or other documentary evidence of illness, injured casualty status or limited duties. Either way, add a prescription from a health care provider that gives the date, your name, a statement that it is for a removable windshield placard, the expected duration of the condition and the provider’s signature, or have your provider complete Section C of the application.

Eligibility

  • You are a veteran or an active duty service member
  • You have a documented service-connected disability that limits or impairs your ability to walk
  • You provide either a health care provider’s prescription for a removable windshield placard or Section C of the application completed by a health care provider
