Your Social Security number, an unexpired government photo ID, employer names, addresses and dates for the last 18 months, your bank account and routing numbers, and for recent military service your most recent DD214, orders to report or release, Leave and Earnings Statement, or W-2. Federal employment in the last two years also needs an SF-50 or SF-8 and pay stubs.
