Gold Star Family Designation on Illinois Driver’s Licenses and ID Cards

If your service member died while serving in the U.S. Armed Forces, in peacetime or war, you can have a Gold Star Family designation added to your Illinois driver’s license or state ID card at no cost. Normal renewal fees still apply. Eligible immediate family are the surviving spouse, a parent, a child or stepchild, and a sibling or half-sibling.

Complete the Affirmation for Gold Star Family Designation form and submit it at an Illinois Secretary of State office.

What you'll need

The completed affirmation form and documentation of the service member’s death in service and your relationship to them.

Eligibility

  • You are an Illinois resident
  • You are the surviving spouse, parent, child, stepchild, sibling or half-sibling of a service member who died while serving in the U.S. Armed Forces
