What you'll need

The report of casualty or death certificate showing a line-of-duty death in support of the Global War on Terrorism, proof of your relationship as next of kin, and proof the service member was an Illinois service member.

Eligibility

  • You are the surviving next of kin of an Illinois service member
  • The service member was killed in the line of duty in support of the Global War on Terrorism
