Domiciliary and long-term skilled nursing care at five state homes

Illinois runs five veterans’ homes offering domiciliary care for residents who need minimal assistance and long-term skilled nursing care, for veterans and their eligible family members. They are in Anna at 792 N. Main Street, Chicago at 4250 North Oak Park Avenue, LaSalle at 1015 O’Conor Avenue, Manteno at 1 Veterans Drive, and Quincy at 1707 N. 12th Street. An honorably discharged veteran’s surviving spouse or Gold Star family member with no adequate means of support who cannot earn a living may be admitted at Anna or Quincy if beds are vacant, after a year of Illinois residency.

How to apply

Complete the Illinois Department of Veterans Affairs application for admission and submit it to the home you want. Admission depends on the home being able to meet your care needs and on space being available.

What you'll need

Your DD214, medical documentation of your disability, disease or wounds, and proof that you entered the service from Illinois or have lived in Illinois for the year before applying.

Eligibility

  • You received an honorable discharge
  • You entered the U.S. Armed Forces as an Illinois resident, or you have been an Illinois resident for the year before applying
  • You served at least one day during a recognized wartime period or in a hostile fire area with a campaign or expeditionary medal, retired for a service-connected disability or injury, served at least 24 months of continuous active duty after September 7, 1980, completed a reserve component federal active duty mobilization, completed 20 years of reserve service and qualify for retired pay, or were discharged for hardship or released early in a reduction in force
  • You have a disability, disease or wounds that leave you unable to earn a living
