Indiana Governor's Memorial Certificate

Indiana Department of Veterans Affairs (IDVA), through the Indiana Veterans Memorial Cemetery
Commemorative certificate signed by the Governor, at no cost

When an honorably discharged veteran dies, their surviving family can request a certificate signed by the Governor of Indiana commemorating their service. It covers service in the U.S. Armed Forces, including the National Guard and the Reserves.

How to apply

Ask your funeral director, a veterans service organization or your county veterans service officer to request it, or complete the application and send it to the Indiana Veterans Memorial Cemetery, 1415 North Gate Road, Madison IN 47250, phone 812-273-9220.

What you'll need

The completed request form and a copy of the veteran’s DD214 or record of military service.

Eligibility

  • You are a surviving family member of a deceased veteran
  • The veteran received an honorable discharge from the U.S. Armed Forces, the National Guard or the Reserves
