How to apply

Apply for your first distinctive plate in person at an Indiana BMV branch, then renew online at myBMV. For eligibility questions contact IDVA at 317-232-3910 or 800-400-4522.

What you'll need

Proof of eligibility for the plate you want, which for most veteran plates is an IDVA-certified Certification of Eligibility for Veterans License Plates (State Form 32584) plus your discharge paperwork.
