State-run long-term nursing, rehabilitation, independent living and memory care

The state home in West Lafayette provides long-term skilled nursing care, short-term rehabilitation, independent living and memory care for veterans and their eligible family members. On-site specialty care covers pulmonology, endocrinology, nephrology, psychiatry, podiatry, dentistry, social services, music therapy and physical, speech, hearing, occupational and respiratory therapies. Residents have a library and media centre, gardens, crafts and hobbies, chapel services, a beauty shop, transport and recreation activities.

Complete the IVH admissions application packet and contact the IVH Admissions and Marketing Department at 765-497-8072, or email admissions&marketing@ivh.in.gov. The home is at 3851 N. River Road, West Lafayette IN 47906, phone 765-463-1502.

What you'll need

Your DD214 showing an honorable discharge, proof you have lived in Indiana for at least a year, medical documentation of your care needs, and your marriage certificate or relationship documentation if you are applying as a family member.

Eligibility

  • You are an honorably discharged veteran of the U.S. Armed Forces, the spouse or surviving spouse of an eligible veteran, or the Gold Star parent of a service member who died while serving
  • You have been an Indiana resident for at least the one year immediately before you apply, or you qualify for an admission waiver because the veteran enlisted from Indiana or was discharged to Indiana
  • You pass the criminal background check the home conducts
