Family members of a service member who died while serving on active duty during a time of military conflict, or who died as a result of that service, can get this plate. One numbered set is free, additional sets cost $25, and you pay a $5 annual registration fee plus your annual registration renewal fee. A personalized set costs $25, additional personalized sets cost $50, with a $5 annual validation fee.
Mail the Special Plate Application G-10 with your documentation and any fees to the Iowa Department of Veterans Affairs, 7105 NW 70th Avenue, Building 3465, Johnston, IA 50131.
Official documentation showing your relationship to the deceased service member, and official documentation that the service member died as a result of their military service.
