You can claim short-term payments that replace part of your wages if you lose your job through no fault of your own. Employers covered by the Iowa Employment Security Law pay for the program entirely, so nothing comes out of your wages.

Your Social Security number; the name, payroll address, phone number and employment dates of your last employer; the amounts of any vacation or severance pay; your alien registration number if you are not a U.S. citizen; your DD214 Member Copy 4 if you served in the last 18 months; an SF8 if you were a federal employee in the last 18 months; and the names, dates of birth and Social Security numbers of the dependents you claim on your federal return.
