Your email address, Social Security number, full mailing address, phone number, county of residence and driver's license number; the name and mailing address of your last employer with start and stop dates and reasons for leaving for the last 18 months; your DD214 Member Copy 4 if you were on active duty in the past 18 months; an SF50 or pay stub if you were a federal employee; your U.S. employment authorization number if you are not a citizen; and the amount of any separation, vacation or holiday pay.
