What you'll need

The service member’s federal active duty orders, the death certificate or DD1300 report of casualty, a line of duty determination, and proof that you are a named beneficiary.

Eligibility

  • You are a beneficiary of a service member who was a Kentucky resident
  • The service member was serving in a reserve component of the U.S. Armed Forces
  • The service member died in the line of duty while serving on federal active duty
