One commemorative medal issued to the primary next of kin

Kentucky issues one medal to the primary next of kin of a Kentucky resident killed in a combat related incident on or after January 1, 1991. Precedence runs surviving spouse, children, parents unless legal custody was granted to someone else, a blood or adoptive relative granted custody, siblings oldest first, grandparents, other relatives, then a close friend or associate.

How to apply

Submit a completed Kentucky Medal for Freedom nomination form with a copy of the service member’s DD1300 report of casualty, by fax to 502-564-9240 or by mail to Kentucky Department of Veterans Affairs, ATTN: Medal for Freedom, 1111B Louisville Road, Frankfort, KY 40601. What you'll need

A completed nomination form, a copy of the service member’s DD1300 report of casualty, and documentation of your relationship to them.

Eligibility

  • You are the primary next of kin of a Kentucky resident who was killed while serving in the U.S. Armed Forces
  • The death occurred in a combat related incident on or after January 1, 1991
  • You are highest in the order of precedence, which runs surviving spouse, children, parents, a relative granted custody, siblings oldest first, grandparents, other relatives, then a close friend or associate
