Your Social Security number, Alien Registration number if you are not a U.S. citizen, date of birth, mailing address and phone number; the name, mailing address, phone number, employment dates and reason for leaving for every employer in the last 18 months; and, if they apply, a list of other states you worked in, the name and address of any temporary agency, an SF8 or SF50 for former federal government work, your DD214 member copy 4 for former military service, and your union information.
