One state honor medal per veteran

You can receive a state medal recognizing your honorable military service. The ribbon colour reflects your service: blue for an honorable discharge, purple for a Purple Heart recipient, black for a prisoner of war, and yellow for a service member killed in action. Only one medal is awarded per veteran.

How to apply

Apply online, or send the completed application and copies of your documentation to the Louisiana Department of Veterans Affairs, ATTN: Honor Medal, PO Box 94095, Baton Rouge LA 70804-9095, or fax 225-219-5590. For help, call 225-219-3700.

What you'll need

Your DD214, or a DD1300 report of casualty if the veteran died before separation, plus proof of the Louisiana residency connection you are claiming.

Eligibility

You are a current Louisiana resident, were a Louisiana resident when you entered the U.S. Armed Forces, or were a Louisiana resident at the time of your death
You served in an active or reserve component of the U.S. Armed Forces
You received an honorable discharge, or died before separation, and can attach a DD214 or DD1300
