Submit the Michigan Office of Retirement Services retirement application for spousal benefits by mail to Michigan Office of Retirement Services, P.O. Box 30171, Lansing, MI 48909-7671, or by fax to 517-284-4416.

What you'll need

A copy of the service member’s death certificate, your birth certificate and your marriage license.

Eligibility

  • You are the surviving spouse of a Michigan National Guard service member
  • The service member qualified for Michigan National Guard state retirement pay
