Michigan Department of Military and Veterans Affairs (DMVA)
Skilled nursing, rehabilitation, memory and palliative care at three state homes

Michigan runs three veteran homes providing skilled nursing care: Michigan Veteran Homes at Grand Rapids (128 beds, 2950 Monroe Avenue NE), D.J. Jacobetti in Marquette (126 beds, 425 Fisher Street) and Chesterfield Township (128 beds, 47901 Sugarbush Road). Each home provides long-term and short-term skilled nursing care, physical, occupational and speech therapy, dementia and memory care with 24-hour supervision, and palliative care. You do not have to be a Michigan resident.

How to apply

Contact the home you want directly to verify eligibility, then apply through the Michigan Veteran Homes admissions page or the online admission inquiry form. Admissions numbers are 616-345-6110 for Grand Rapids, 906-225-6889 for D.J. Jacobetti and 586-210-7103 for Chesterfield Township.

What you'll need

Your DD214 showing an honorable discharge, documentation that you are eligible for VA health care or VA financial assistance for long-term nursing care, and your medical information. A family member applying also needs proof of their relationship to the veteran.

Eligibility

  • You are an honorably discharged veteran who is eligible for VA health care or VA financial assistance for long-term nursing care, or you are the spouse, surviving spouse, former spouse or Gold Star parent of a veteran who is eligible or who was eligible when they died
  • You need the level of nursing or memory care the homes provide
