Minnesota Gold Star Family License Plate

Plates issued free of charge

Gold Star Family license plates are free of charge for the surviving spouse, parents, children or siblings of a service member who died while serving on active duty in the U.S. Armed Forces. A motorcycle version is available.

How to apply

Complete the Minnesota Department of Public Safety Gold Star License Plate Application and take or mail it to Driver and Vehicle Services with your documentation. This plate has its own application rather than the general special plate form.

What you'll need

A copy of the DD1300 report of casualty or the DD2064 certificate of death, or certification of eligibility from the Minnesota Commissioner of Veterans Affairs.

Eligibility

  • You are the surviving spouse, parent, child or sibling of a service member who died while serving on active duty in the U.S. Armed Forces
  • You can provide a DD1300 report of casualty, a DD2064 certificate of death, or certification of eligibility from the Minnesota Commissioner of Veterans Affairs
