Skilled nursing and long-term care at seven state veterans homes

Minnesota runs seven veterans homes offering health care and a safe environment for veterans and their spouses: Bemidji, Fergus Falls, Luverne, Minneapolis, Montevideo, Preston and Silver Bay. They provide skilled nursing care, specialized dementia care, medical services with 24-hour nursing care, social services, recreational activities, physical, occupational and speech therapy, mental and behavioral health services, chaplain and spiritual services, nutritional services, pharmaceutical services, an oral health program, transportation to local appointments, barber and beauty services and community involvement.

How to apply

Submit the Minnesota Veterans Homes Application with the documentation listed on it to the home you prefer, in person, by mail, or by email to NewHomesAdmissions@state.mn.us.

What you'll need

Your DD214, proof of Minnesota residency, a marriage certificate if you are applying as a spouse, and the medical documentation listed on the application.

Eligibility

  • You are a Minnesota resident
  • You received an honorable discharge after serving 181 consecutive days on active duty in the U.S. Armed Forces, or were discharged earlier because of a service-connected disability received in the line of duty, or you are the spouse of an eligible veteran and are at least 55 years old
  • You require skilled nursing care
