A state-issued driver’s licence or ID card, your mailing address and phone number, your Social Security number, the name and address of each employer in the past 18 months with start and end dates and reason for separation, pay stubs showing year-to-date earnings from each, DD214 Member Copy 4 if you separated from the U.S. Armed Forces in the last 18 months, an SF50 or SF8 if you separated from federal employment in the last 18 months, and your alien registration or visa number if you are not a U.S. citizen.
