Honorably discharged Missouri resident veterans with a service-connected disability can get one free set of Disabled Veteran license plates. If you want the wheelchair accessibility symbol on the plate, a physician has to sign a statement for you as well.

How to apply

Submit the Application for Missouri Military Personalized License Plates, Form 4601, with a VA letter dated within the last six months verifying your service-connected disability. Add the signed Physician's Statement for Disabled License Plates and Placards, Form 1776, if you want the wheelchair accessibility symbol.

Eligibility

  • You are a Missouri resident
  • You received an honorable discharge from the U.S. Armed Forces
  • You have a service-connected disability
  • You can provide a VA letter dated within the last six months verifying the disability
  • If you want the wheelchair accessibility symbol, a physician has signed Form 1776 for you
