Complete and sign the Application for Missouri Military Personalized License Plates, Form 4601, and mail it with copies of your proof of eligibility and the fee shown on the front of the application to the Motor Vehicle Bureau, 301 West High Street Room 370, PO Box 569, Jefferson City MO 65105-0569. Proof of eligibility for the plate you want: a Form WD AGO 53-55, an official discharge order from the U.S. Armed Forces, general orders, a Form NGB 22, a DD214 or DD215, or a current military ID card.
