Family members of a service member who was killed in action or declared missing in action can be awarded a state medallion. The first medallion is free and any additional ones cost $20.

Mail the completed application and your supporting documentation to Montana Veterans Affairs Division, Attn: Honor and Remember Medallion, 1956 Mt Majo Street, PO Box 5715, Helena MT 59604-5715. For questions call 406-324-3742.

What you'll need

Official documentation verifying the service member’s death or the date they were declared missing in action, and proof of your family relationship.

Eligibility

You are the surviving spouse, parent, adoptive or foster parent in loco parentis, child, stepchild, adopted child, sibling, half sibling or grandparent of the service member
The service member was a Montana resident serving on active duty in the U.S. Armed Forces
The service member was killed in action or declared missing in action on or after September 8, 1939 in an operation recognized by the Department of Defense
