You can claim temporary income if you lose your job through no fault of your own. It replaces part of your lost wages while you look for work, is not meant to be a permanent income, and employers pay all the costs.

The business name, address, phone number and employment dates for every employer in the last 18 months; your bank account and routing number if you want direct deposit; the name and number of your union hall if you were employed through a union; your DD214 Member Copy 4 if you left the U.S. Armed Forces in the last 18 months; an SF-8 or SF-50 if you left federal employment in the last 18 months; and your alien registration card number and expiration date if you are not a U.S. citizen.
