Apply online with the World War II Recognition Medal Program form, uploading your discharge documents, or mail the State of Nebraska WWII Recognition Medal Application with a copy of your discharge documents to the Nebraska Department of Veterans' Affairs, ATTN: WW2 Program, PO Box 95083, Lincoln NE 68509-5083. What you'll need

A copy of your DD214 or other discharge paperwork as proof of eligibility.

Eligibility

  • You served in the U.S. Armed Forces between December 1, 1941 and December 31, 1946
  • You are a current Nebraska resident, or you were a Nebraska resident when you entered the U.S. Armed Forces
  • You were discharged under conditions other than dishonorable
