Submit the Application for Military Skills Test Waiver, certified by your commanding officer, with a copy of your Military Qualification Record and Military License at any New Hampshire Division of Motor Vehicles office, or mail it to New Hampshire Division of Motor Vehicles, 23 Hazen Drive, Concord, NH 03305.

• You are a New Hampshire resident service member or former service member applying for a commercial driver license
• You were licensed in the past year on a military vehicle similar to the commercial vehicle class you are applying for
• You have not held more than one license, other than a military license, in the past two years
• You have not had any license suspended, revoked or cancelled in the past two years
• You have no disqualifying convictions, no more than one conviction for a serious traffic violation, no conviction for a traffic control violation resulting from an accident, and no record of an accident in which you were at fault
