Your Social Security number, Alien Registration number if you are not a U.S. citizen, New Jersey driver licence or non-driver ID number, pension information, the amount and duration of any separation pay, your recall date if you expect one, union hiring hall details if you work through a union, your DD214 if you served in the U.S. Armed Forces in the last 18 months, Form SF-8 or SF-50 if you are a former federal employee, bank account and routing numbers for direct deposit, and the name, address, phone number, occupation, employment dates and reason for separation for every employer in the last 18 months.
